Candice Hamre, an advanced practitioner since 2022, provides primary care to patients of all ages across all areas. Before becoming a Nurse Practitioner, Candice was a Registered Nurse for nearly a decade with experience in caring for patients in outpatient and inpatient settings with a heavy emphasis on labor and delivery and newborns. This is where Candice’s passion for women’s health and pediatrics comes from, although she enjoys caring for patients from all walks of life. Having obtained her Bachelor of Science in Nursing and master’s in nursing leadership and Education from Grand Canyon University, she then pursued further education at Chamberlain University where she graduated and became a certified Family Nurse Practitioner. Candice is married to her high school sweetheart Edward, whom she has four children with. Outside of caring for patients, Candice can be found with her family attending school, church, or community functions, or simply spending quality time with local friends and family.
Dustin began his healthcare career in 2006 working as an ER secretary, then an ER Registered Nurse in a busy Oklahoma City emergency department. He has worked as a Family Nurse Practitioner (FNP) since 2014 after graduating from Midwestern State University located in Wichita Falls Texas. His FNP experience varies between emergency department, urgent care, telemedicine, and family practice. In addition, he serves as an adjunct professor at a nearby university. In his current role at Cohesive Family Medicine, he particularly enjoys serving individuals of all backgrounds and enjoys caring for a variety of conditions. In his time away from work, Dustin enjoys spending time with his son Brennan, while enjoying the outdoors, and woodworking.
A family nurse practitioner for over eight (8) years, Alicia is a founding member of the Acclaimed (now Cohesive) provider staff. Having obtained degrees in Biology and Nursing, she pursued further education at Texas Tech University Health Sciences Center where she graduated with her master’s in nursing and became a certified Family Nurse Practitioner. Alicia has a combined thirteen (13) years of healthcare experience caring for patients in hospital emergency rooms, long-term care facilities, and outpatient clinics. As a family practice provider, she enjoys the variety and serving patients in all walks of life. She especially has a heart for children and pediatric care as well as women’s health.
During her time off, Alicia often spends time with her husband, Jared, and two sons. Together they enjoy staying active outdoors.
Keith A Conaway, MD has been a board eligible family physician since 1996, having graduated from Oklahoma University Medical School in 1993 and the OU Department of Family Medicine in 1996. He resides and has practiced in Shawnee, Oklahoma since 1996, and has vast experience in the clinical, as well as hospital setting. He enjoys seeing patients of all ages from newborns to geriatrics and has a special affinity for the elderly population.
Outside of medicine, Dr. Conaway enjoys traveling with his wife Brandi, both locally and internationally, and especially loves Europe. He particularly enjoys his family of four grown kids and their spouses and is soon to be a grandfather to twins! His new hobby will be to excessively spoil his grandkids, while annoying his kids.
